福尔马林固定及石蜡包埋病理材料电镜检查的技术和应用
TECHNIQUE AND APPLICATION OF FORMALIN FIXED OR ITS PARAFFIN EMBEDDED MATERIAL FOR ELECTRON MICROSCOPY
【摘要】 <正> 超微结构病理从亚细胞水平反映了病变的特点和性质,较之光学显微镜(光镜)下的细胞病理有了很大的提高,两者结合应用可以大大提高病理诊断水平,特别是肿瘤病理诊断。当然大多数肿瘤的病理诊断用光镜检查能得到解决,但仍有一些(约8%)较疑难的病例需要配合电子显微镜(电镜)才能作出诊断,尚有3%病例则需要依靠电镜检查才能确诊。足见电镜检查对解决疑难诊断是能起到一定的作用。但一般常规病理标本大多已经过福尔马林固定,石蜡包埋,亚微结构受到一定程度的影响,能否再进行回顾性电镜观察,应用于病理诊断,是病理工作者迫切需要解决的问题,近年来不少学者已作了可喜
【Abstract】 It wag estimated that about 8% of routine pathological specimens could be diagnosed only by collaboration of light microscopy and electron microscopy. But conventional pathological specimens were usually fixed in formalin or embedded in paraffin. Were these materials still suitable for electron microscopy? It was the purpose of this study to find some convenient method in treating those routine pathological materials that were already fixed in formalin or embedded in parattin for electron microscopy. Materials of 82 oases were studied. Our experiences in preparation of tha specimens and application of these materials were discussed in detail. 1.Technique:Early fixation in buffered formalin (pH7.2 osmotality 200~400 mOsm/L) were of prime importance in .good preservation of ultrastruotures. In order to render conventional pathological specimens suitable for ultrastruotural observation, it was suggested to use neutralized formalin as routine fixatives.Deparaffinization of the paraffin embedded materials by using chloroform and ethanol. After deparaffinization the tissues were washed in 0.1M oacodylate buffer containing 7% sucrose (pH 7.2, 350 mOsm/L) overnight. An then they were fixed in 1% buffered osmium tetroxide and processed in routine manner for ultrathin sections.2.Applications Ultra-thin sections prepared from the formalin fixed or paraffin embedded materials by the methods mentioned above usually showed fairly well preserved ultrastructure for diagnostic clues, such as special cell junctions, miorovilli, secretory granules, filaments, immune complexes, infectious agents etc.The oldest specimen in this series was already preserved in formalin for more than 2 decades or embedded in paraffin for more than 4 years in room temperature. In spite of long periods of preservation the ultrastruotures were still fairly well preserved which would be not only very contributing to solve difficult diagnostic problems but also providing valuable information to the study of pathogenesis and morphogenesis of the lesions.
