实时二维超声心动图的临床应用
CLINICAL APPLICATION OF REAL-TIME TWO-DIMENSIONAL ECHOCARDIOGRAPHY
【摘要】 <正> M 型超声心动图在临床上推广使用已有20多年的历史,近几年国内亦日益普及,目前已成为研究各种心脏病的一种重要工具。M 型超声心动图检查为无创性,具有结果准确、可以重复,检查费时较短以及可在床边进行等优点。其主要缺点是仅能提供心脏的一维图像,所显示的心内结构与心脏解剖不同,使临床医生不易理解,并且不能观察心内结构的空间方位。
【Abstract】 Two-dimensional echocardiography(TD Echo)is a new diagnostic techniquerapidly developed in the last few years.It has been applied clinically in our hospitalsince the beginning of 1980.A mechanical real-time sector echocardiograph(ATL)with a 80°-90°sectoring transducer head(3MHz),providing 30 frames/sec was usedin this study.It has good resolution ability and can scan as deep as 21 cm.Last year,350 cardiac cases were examined.We paid special attention to thecorrelation of the preoperative echocardiographic features and the operative findings,and found there was a fairly close agreement.In this paper,we report:(1)the routine scanning positions and various tomo-graphic planes in our TD Echo examination and the modified technique for certainrare cardiac anomalies;(2)the two-dimensional echocardiographic characteristics ofvarious heart diseases.TD Echo was found to be particularly valuable in the diagnosis of valvularheart disease not only in the assessment of the type of valvular lesions but also inidentifying the characteristics and degree of valvular damage and the size of theinvolved valvular ring.All these were important for the selection of surgical casesand the choice of surgical approach.This technique was also valuable for the dia-gnosis of congenital heart diseases.The intracardiac abnormalities could be visua-ized and their degree could be estimated,so that invasive cardiac catheterization andangiocardiography might be substituted by TD Echo in some cases.In the discoveryof intracardiac tumors,such as atrial myxomas,intracardiac thrombi,vegetations,ventricular aneurysms,idiopathic cardiomyopathies,and pericardial effusion,TDEcho has also displayed its superiority.
