猫的内脏牵拉痛反应及电针效应

【摘要】 <正>我们在临床胃大部切除术中,通过多种生理指标,观察到内脏牵拉痛反应的表现非常复杂,可分为内脏痛反应和内脏牵拉反应,交错混合出现。为了进一步分析其发生原理和克服这些反应,本文用猫同时记录5种生理指标的变化,分别膨胀胃壁和牵拉胃周围组织,以模拟内脏牵拉和内脏痛,观察其反应,着重分析两种反应的表现和传入途径,并探讨其电针效应,为进一步克服内脏牵拉痛反应和研究针麻原理提供资料。

【Abstract】 Experiments were carried out on 56 cats immobilized with Flaxedil. The femoral arterial blood pressure, the ear plethysmogram, the size of the pupils, the efferent discharges of the recurrent laryngeal nerve and the movement of the ducdenum were continuously taken simultaneously with a polygraph. Imitation visceral pain readings and visceral dragging reactions were performed by distension of the gastric wall and dragging of the tissues arround the stomach so as to study the manifestations of these two types of reaction and their afferent pathways. The effect of electro-acupuncture was also examined. The conclusions were:1.Distension of the stomach caused elevation of the mean blood pressure, decreased amplitude of the plethysmcgram, dilated pupils and increased efferent discharges in the recurrent laryngeal nerve. These reactions could be inhibited by the injection of Dolantin. Thus it was suggested that these responses were concerned with the visceral pain, and it was possible to consider that the results of distending the stomach was mainly a reaction of visceral pain.2. Stretching the stomach induced increased movements of the duodenum and increased efferent discharges in the recurrent laryngeal nerve. Such a reaction could be elicited by the injection of emetic apomorphine and reduced by the injection of an antiemetic. This would suggest that these responses might reflect the visceral dragging reaction, and it was possible to consider that reaction caused by stretching the stomach was mainly a visceral dragging reaction.3. After bilateral splanchnic nerves were cut the stomach distension reaction obviously was reduced, while stimulation of the proximal end of the splanchnic nervepsoduced a reaction analogous to that of distensin of the stomach. So it was suggested that the main afferent pathway of visceral pain is the splanchnic nerve.4.Bilateral vagotomy usually brought forth a reduced stretching reaction of the stomach, but a similar dragging reaction appeared when the proximal end of the vagus was stimulated. It was suggested that the main afferent pathway of visceral dragging reaction is the vagus nerve.5.Reaction of visceral dragging pain could be inhibited by electro-acupuncture of "Naiguan" point or of the median nerve.
