尿雌三醇简捷的荧光测定法在高危妊娠的应用

【摘要】 <正> 对于患有威胁胎儿宫内发育疾病的孕妇,判断其胎儿健康情况,选定适宜时间分娩以减低婴儿围产期死亡,是产科工作中一项重要课题。孕妇尿中雌激素是胎儿体内雌激素的前驱物质,经胎盘转化为雌三醇自母体肾脏排出。近20多年很多文献报告关于探索简易测定孕妇尿雌三醇作为估计胎儿宫内健康情况的指标。我们采用了北医生理教研组提出的改良Bra-mhall氏的尿雌三醇快速荧光测定法稍加改进,一年多来用于高危妊娠测验胎儿健康情况,认为是协助临床处理的一种简捷、可信赖的试验方法。

【Abstract】 Using a modified fluorometric assay of 24-hrs urinary estriol(E3), 101 urinary estriol(E3) estimations in 57 high-risk pregnancies were performed from Aug. 78--Sept. 79.Among which: severe toxemic pregnancies 24, hypertension complicated with toxemias 10,prolonged pregnancies 11, diabetes and other complications 12. All of the specimens werecollected in the third trimester. Compared the urinary E3 level of normal late pregnancies with the E3 level of thishigh--risk group, the later showed a marked low values. Estriol excretion of 24hrs urine was<12mg in 46 of 101 estimations (45.5%). The low estriol level showed nearly a hori-zontal line without a normal elevated values in late pregnancy complicated with toxemiasand hypertensive cases. The birth weight of neonates appeared to be closely correlated with the urinary E3 va-lue. In the 57 high--risk pregnancies, the urinary E3 estimation obtained within 1 wk. befordelivery, values showed above 12mg/24hr in 35 pts., and there were no fetal loss. However,urinary E3 values below 12mg/24hrs. in 21 cases, there were 2 fetal death and one neona-tel death. The E3 values obtained within 1 wk. before delivery of these 3 cases were 5.1mg, 4.3mg and 11. 8mg respectively. The authors concluded that this improved Fluorometric method for urinary estriol mea-surement provided useful prognostic information in high-risk pregnancy.
