间接免疫荧光用于病毒性下呼吸道感染多病原的鉴别

ETIOLOGIC DIFFERENTIATION IN LOWER RESPIRATORY TRACT VIRUS INFECTIONS BY INDIRECT IMMUNOFLUORESCENCE

【摘要】 在50例病毒性肺炎幼儿的鼻咽部剥落细胞内,检测出24例有3或7型腺病毒抗原(48%),在其中2例的肺组织内也检测出同样病毒的抗原,并分离出7型腺病毒;5例有呼吸道融合细胞(RS)病毒抗原(10%);3例有Ⅰ、Ⅱ、或Ⅲ型副流行性感冒(副流感)病毒抗原(6%),其中1例的肺组织内也检测出同样病毒的抗原,并分离出Ⅰ型副流惑病毒。在7例毛细支气管炎婴儿的鼻咽部剥落细胞内,检测出3例有3或7型腺病毒抗原,一例有RS病毒抗原。在一例胸膜炎儿童的鼻咽部剥落细胞内,检测出副流感病毒抗原。在11例非呼吸道感染的儿童中,检测这三种病毒抗原的结果都是阴性。在39例病毒性肺炎中,抗7型和抗3型腺病毒的红血细胞凝集抑制抗体升高4倍以上的各为39.5%和17.9%,共为53.8%;抗RS病毒的中和抗体和抗Ⅰ型副流感病毒的红血细胞吸附抑制抗体升高4倍以上的都是10.3%。在5例毛细支气管炎中,抗7型腺病毒、抗3型腺病毒和抗RS病毒的抗体升高4倍以上的分别为1例、1例和2例。在10例非呼吸道感染,抗这三种病毒的抗体都没有升高。病毒抗原检测同双份血清内抗体测定的结果基本相符。

【Abstract】 Prototype strains of types 3 and 7adenoviruses, respiratory syncytial virusand types Ⅰ,Ⅱ andⅢ parainfluenzaviruses were conjugated with correspon-ding types of immune sera and detectedby indirect immunofluorescent technique.Specific fluorescence was observed inthem, but not in the controls. Crossstaining gave negative results. One-stepinhibition test showed that specific fluo-rescence could be inhibited. Of the 50 young children diagnosedclinically as virus pneumonia, adenovirusantigen of type 3 or 7 was detected inexfoliated nasopharyngeal cells in 24(48%),the same virus antigen also de-tected in and Type 7 adenovirus isolatedfrom the lung tissue in 2 of them;re-spiratory syncytial virus antigen in 5(10%);and parainfluenza virus antigenof type Ⅰ, Ⅱ orⅢin 3 (6%),the samevirus antigen also detected in and TypeⅠ parainfluenza virus isolated from thelung tissue in Ⅰ of them. Of the 7 in-fants diagnosed clinically as bronchioli-tis, adenovirus antigen of type 3 or 7was detected in exfoliated nasopharyn-geal cells in 3, and respiratory syncytialvirus antigen in 2. Parainfluenza virusantigen of type Ⅰ,Ⅱ orⅢ was detectedin exfoliated nasopharyngeal cells in 1child diagnosed clinically as pleuritis.In 11 children with diseases other thanrespiratory tract infections, the resultsof detection for these 3 kinds of virusantigen were all negative. Of the 39 cases of virus pneumonia,a 4 fold or greater rise in hemagglutina-tion-inhibiting antibody to Type 7 andto Type 3 adenovirus was demonstratedin 39.5% and 17.9% respectively, witha total of 53.8 %;a 4 fold or more in-crease in neutralizing antibody to respi-ratory syncytial virus and a 4 fold orgreater increase in hemadsorption-inhibi-ting antibody to Type Ⅰparainfluenzavirus were shown in 10.3% for both. Ofthe 5 cases of bronchiolitis, a 4 fold ormore rise in the antibody to Type 7 ade-novirus, to Type 3 adenovirus and torespiratory syncytial virus was demons-trated in 1 case, 1 case and 2 cases re-spectively. No elevation of antibodies tothese 3 kinds of virus was observed in10 children with diseases other than re-spiratory tract infections. Comparison of the detection of viralantigens by indirect immunofluorescencewith the assay of antibodies in pairedsera showed a rather good agreement.
