【Abstract】 The Observations that we have made concerning paragonimiasis in the central nervous system are based upon the study of 76 patients admitted to the Neurological Service of the First Shanghai Medical College in the course of the last 5 years. They comprise 68 instances involving the brain, 5 instances involving the cord and 3 instances involving both brain and cord. Clinical features, roentgenologic findings, surgical treatment and results of these cases are discussed. Evidence collected from clinical material and pathological specimens of the present series seems to indicate that the central nervous system involvement in paragonimiasis arises as a result of direct invasion of the worm rather than egg embolism. There is no evidence to support the latter assumption. The authors concur with the view that an adult worm is able to migrate inside the host, and we also strongly believe that in case of brain involvement the path of entry of the worm is through the carotid canal. Anatomical proximity of the canal to the frequent site of brain lesion is pointed out. Clinical classification of paragonimiasis of the central nervous system is suggested. Indications and contraindications of surgical treatment are outlined. The authors believe that the main purpose of an operation is to try to get rid of the worm.更多还原
