基于症状单元临床特征的证候要素诊断要点研究

【摘要】 目的:基于症状单元临床特征研究证候要素诊断要点。方法:提出证候单元假说,认为证候单元是能够独立存在的最小证候。从历代医案数据库中提取了五脏相关证候单元。从60种客观兴趣度评价方法(评价规则相关性的数学方法)中选择了一种最适合于分析证素与症状之间关系的方法——φ相关。应用该方法找出证候单元及证候要素对应的特异症状单元,并提取了证候要素对应特异症状单元的临床特征。应用φ相关对证候要素与症状单元的临床特征之间的关系进行研究。结果:从历代医案数据库中提取了29个五脏相关证候单元,涉及20个证候要素。运用φ相关分析方法找出了29个证候单元和20个证候要素对应的特异症状单元。找出了20个证候要素对应特异症状单元的部位特征和性质特征。研究了证候要素与症状单元临床特征的相关性,最后找出了部位证素对应的特异部位特征和性质证素对应的特异性质特征。结论:证候单元对应于症状单元,证候要素对应于临床特征。部位证素和性质证素可以通过其对应的临床特征进行区分。部位证素对应于症状单元的部位特征,性质证素对应于症状单元的性质特征。证候单元假说是梳理历代医案数据库中证候的一种新方法。基于症状单元的临床特征可以得出证候要素更为简洁的诊断要点。
【Abstract】 Objective: Extracted diagnostic points of syndrome elements based on the clinical characteristics of symptomatic units.Methods: Proposed a hypothesis about syndrome unit. Syndrome unit is regarded as the smallest syndrome that can exist independently. Syndrome units of five Zangs were extracted from the clinical record database.φcoefficient was selected from 60 interestingness measures as the best measure for evaluate the correlation between syndrome elements and symptoms. Specific symptomatic units corresponding to every syndrome element and every syndrome unit were extracted using this measure. Clinical characteristics of specific symptomatic units corresponding to every syndrome element were extracted. The correlation of syndrome elements and clinical characteristics of symptomatic units were studied using the measureφcoefficient.Results: 29 syndrome units of five Zangs were extracted from the clinical record database of past dynasties. Specific symptomatic units corresponding to 20 syndrome elements and 29 syndrome unit were extracted usingφcoefficient. Position characteristics and nature characteristics of specific symptomatic units corresponding to every syndrome element were extracted. The correlations of syndrome elements and clinical characteristics of symptomatic units were studied. Specific position characteristics corresponding to position syndrome elements and specific nature characteristics corresponding to nature syndrome elements were extracted.Conclusions: Syndrome units are corresponding to symptomatic units and syndrome elements are corresponding to clinical characteristics. Position syndrome elements and nature syndrome elements can be differentiated by their relationship with clinical characteristics of symptomatic units. Position syndrome elements are corresponding to position characteristics of symptomatic units while nature syndrome elements are corresponding to nature characteristics of symptomatic units. The hypothesis of syndrome units will facilitate the standardization of the syndrome description in the clinical record database of past dynasties. More concise diagnostic points of syndrome elements could be extracted based on clinical characteristics of symptoatic units.
