【摘要】 腭成形术的主要目的是重建软腭肌肉的结构和功能，达到腭咽闭合，为获得正常的语音效果创造条件。手术是实现这一目的的主要手段。然而手术又可造成腭裂患者继发性上颌骨及牙弓生长发育畸形，使腭裂修复手术的利、害关系并存，并一直被认为是一对不可调和的矛盾。如何能避其害而就其利，最大限度的发挥腭裂修复手术的目的，一直是临床医生努力追求的目标。为了提高腭裂修复术后恢复正常语音效果的成功率，就必须深刻认识腭裂的畸形特征，详细研究手术方法的科学性和生物安全性，使支配软腭上提运动的肌肉正确后退复位并行使其功能。为了避免或减少腭裂术后上颌骨生长发育畸形的不利影响，则需在阐明腭裂术后上颌骨生长发育畸形病理结构的基础之上，通过术式设计与方法的应用而予以消除，使腭裂手术成为既能为恢复正常的语音创造条件，又可避免对上颌生长发育产生抑制影响的理想整复方法。在对当今国际流行的腭裂修复方法广泛应用和深入剖析的基础上，我们认为Sommerlad腭帆提肌重建腭裂整复方法具有同时兼顾恢复腭裂患者正常的语音与防止颌骨生长发育畸形的可能，是最适合中国国情、最具发展潜力和应用开发价值。为丰富该方法的生物学基础，客观准确的评价和开发该方法的临床应用价值，为此，我们开展了以下系列研究： 1．本课题通过动物实验与临床研究了解软腭肌肉(主要是腭帆提肌)解剖对软腭组织结构和功能的影响，同时应用主观语音评估与X线头颅侧位片纵向研究腭帆提肌重建术后腭咽功能与语音效果的情况，初步探更多还原

【Abstract】 The functional goals of cleft palate surgery are to result in velopharyngeal competence and facilitate normal speech by restablishing the function of the levator veli palatini . But there are always maxillary and dento-alveolar deformity after primary palatoplasty . It is difficult for plastic surgeon to accomplish the basic goals of achieving good speech results without interfering with the facial growth in the treatment of cleft palate repair . So, it is very importment for us to study the anatomy of the cleft and analyze the scientific and physiological characteristics of every method for cleft palate repair in order to improve velopharyngeal function . Now , it has been regarded that reconstruction of an adequately functioning , soft palate with levator veli palatini sling are more crucial for obtaining the desired speech results . In another way ,every surgeon must explore the causes of this disturbed maxillary and dentoalveolar growth and think about how to prevent this kind of malformation . At present, it is generally accepted that the bone denudation and scar tissue on palate are the most important factors impacting on maxillary growth .
