【摘要】 本课题通过临床与实验研究,系统观察肾白宁水丸防治糖尿病肾病的疗效,探讨糖尿病肾病的中医发病机制及其肾白宁的作用机理。 将临床中的67例早期DN患者随机分为肾白宁治疗组和洛汀新（盐酸苯那普利）对照组,观察尿蛋白、UAER、尿β₂—MG、FBG、HbA₁c、Ccr、TC、TG、LDL、血流变学等项指标及临床症状和体征的改变情况。实验研究,采用STZ诱发DM早期肾病的大鼠模型,观察肾白宁对其尿蛋白排出量、Ccr、FBG、HbA₁c、TC、TG、LDL、实验鼠的肾重量及体重量的变化情况;采用免疫组化（SP三步法）、原位杂交法及彩色病理图像分析系统,定性与定量检测肾组织TGF-β1mRNA、MMP-2mRNA、TIMP-2mRNA及蛋白的表达情况;以透射电镜观察肾组织的结构;探讨肾白宁的作用机理。 结果显示:肾白宁水丸治疗后使早期DN患者临床症状和体征明显改善,尿蛋白、UAER、尿β₂—MG均较治疗前下降,Ccr、血脂、血流变学等项指标明显改善;肾白宁可减少早期DN大鼠的尿蛋白量,使过高的Ccr水平有所下降,使血脂的各项异常指标得到调整,能够稳定血糖水平,增加实验鼠体重量,改善肾重/体重的比值,下调TGF-β1mRNA、TIMP-2mRNA的面密度及PU值,同时上调MMP-2mRNA的面密度及PU值。 肾白宁通过减少UAER、尿β₂-MG,改善Ccr及异常血脂、血流变学相关指标,可保护肾功能,延缓ESRD;可能通过抑制肾组织中TGF-β1mRNA的刺激,纠正MMP-2/TIMP-2系统的功能紊乱,影响肾小球细胞外基质的质和量,使肾脏异常结构得到改善,延缓肾小球硬化。 提示:肾白宁水丸的疗效与剂量和作用时间呈正相关,其远期疗效更为明显。更多还原

【Abstract】 We observed systematically curative effect of preventing and treating diabetic nephropathy （DN） by Shenbaining Water Pill through clinical and experiment researches. We inquired into mechanism of traditional Chinese medicine （TCM） on DN and mechanism of Shenbaining Water Pill on preventing and treating DN.67 patients with incipient DN were randomly divided into treatment group of Shenbaining and contrast group of Luodingxin（benazepril）.We observed two groups the index of urinary albumin, urinary albumin excretion rate（UAE）, urinary β₂-MG, fasting blood glucose（FBG）, glycosylated hemoglobin (HbA_1c), creatinine clearance rate （Ccr）, total cholesterol（TC）, triglyceride（TG）, low density lipoprotein（LDL）, blood rheology and the change of clinical symptom. In the part of experiment, we used DN rat models induced by streptozotocin（STZ）, and observed the change of urinary albumin, Ccr, FBG, HbA_1c, TC, TG, LDL, kidney weigh and body weigh of experimental rats.
