下调脑区的激活及复健片作用机制研究

【摘要】 临床实践中发现,缺血性中风后遗症病人即使在罹病数年后,经治疗仍可出现功能恢复,其机制与下调脑区的激活有关。本文结合近年来神经科学发展的相关理论,对下调脑区激活机制进行了研究。认为发生脑梗死后,由于原先与梗死区发生密切联系的正常脑功能区与梗死区不再有信息联系(不再接受梗死区发出的信息,也不再有信息传至梗死区),致使活动减少,代谢降低,脑血流量相应减少,突触活性也显著下降(上述病理生理改变在梗死后的数年仍可存在)。但这些活动下调的脑功能区——“下调脑区”与原梗死区脑功能是有关的,若能使下调脑区的活动再次激活,对神经功能的恢复将产生影响。下调脑区的激活为中风后遗症的治疗提供了新的思路。因此,课题在充分论证并成功地建立了大鼠缺血性中风后遗症模型的基础上,运用导师治疗中风后遗症的效验方剂——复健片,从神经行为评价,局部脑血流量(rCBF)测定,神经可塑性变化以及脑微血管内皮细胞相关蛋白表达等方面进行了实验研究。结果表明,复健片对缺血性中风后遗症模型神经功能有显著改善作用,而激活下调脑区是其治疗缺血性中风后遗症的重要机制。

【Abstract】 Clinical practice has demonstrated that the patients of sequela of ischemic cerebrovascular disease may recover after treatment even though suffering from it for many years, which is relevant to the activation of the cerebral area of functional down-regulation.The thesis has investigated the mechanism of activation of the cerebral area of functional down-regulation combined with the recent development of neuroscience. It is believed that the activities and metabolization of the cerebral area decrease and the cerebral blood flow reduce and the activities of synapse also decrease remarkably (These pathophysiologic changes will still exist for many years since the infarction), because the cerebral area that was functionally closely linked with the cerebral infarct area formerly has no more communications with the infarct area after stroke. But the function of the cerebral area of functional down-regulation is relevant to the former function of the infarct area. If it is activated again, they will influence the recovery of nervous function. Activating the cerebral area of functional down-regulation provides a new therapeutic method. Therefore, based on the model of rat of sequela of ischemic cerebrovascular disease which has been adequately demonstrated and successfully set up, we apply the tutor’s effective medicine- Fujian tablet, and make experimental study in these respects as follows: neurobehavioral assessment; measuring local cerebral blood flow; variance of the nerve plasticity and the expression of the protein relevant to the endodermis cells of cerebral microvessels, etc. The results show that the Fujian tablet could remarkably improve the nerve function of the model of rats and activating cerebral area of functional down-regulation is the important therapeutic mechanism.
